MONITORING AND REPORTING (continued) <br />Reoottine to Municioalit <br />Any permitted facility discharging to a municipal storm sewer shall provide the municipality with a copy ofthe permit application, <br />upon request. A copy ofthe SWMP shall also be provided to the municipality upon request. <br />Special Monitorine <br />a. TDS monitoring is only required for facilities discharging in the Colorado River Basin. TDS maybe measured as electrical <br />conductivity where asatisfactory correlation with TDS has been established based upon a minimum of five (5) samples. See <br />the individual Certification Rationale for a determination as to whether monitoring is required. <br />b. Phosphorus monitoring is required only for facilities which discharge into the following drainage basins: Cherry Creek <br />basin, Chatfield Reservoir downstream ofthe USGS gage at Waterton and on Pltmt Creek, Dillon Reservoir basin (i.e., Trn <br />Mile Creek, Snake River, Blue River, all tributaries to the Dillon Reservoir), and Bear Creek basin. See the individual <br />Cerification Rationale fw a determination as to whether montloring is required. <br />9. Sienatorv Remuirements <br />a. All repots required for submittal shall be signed and certified for accuracy by the permittce in accordance with the following <br />criteria: <br />I) In the case of corporations, by a principal executive officer of at least the level ofvice-president or his or her duly <br />authorized representative, ifsuch representative is responsible for the overall operation of the facility from which the <br />discharge described in the form originates; <br />2) In the case of a partnership, by a general partner; <br />3) In the case of a sole proprietorship, by the proprietor, <br />4) In the case of a municipal, state, or other public facility, by either a principal executive officer, ranking elected <br />official, or other duly authorized employee. <br />b. Changes to authorization. If an authorization under paragraph a) of this section is no longer acctuate because a differern <br />individual or position has responsibility for the overall operation of the facility, a new authorization satisfying the <br />requirements of paragraph a) of this section must be submitted to the Division, prior to or together with any reports, <br />information, or applications to be signed by an authorized representative. <br />c. Certification. Any person signing a document undo paragraph a) ofthis section (except for [he SWMP certification) shall <br />make the following certification: <br />"I certify under penalty of law that this document and all attachments were prepared under my direction of supervision in <br />accordance with a system designed to assure that qualified personnel properly gather and evaluate the information submitted. <br />Based on my inquiry ofthe person or persons who manage the system, w those persons directly responsible for gathering the <br />information, the information submitted is, to the best of my knowledge and belief, true, accuratq and complete. 1 am aware <br />that there are significant penalties for submitting false information, including the possibility of fine and imprisonment for <br />knowing violarions." <br />E. DEFINITIONS <br />1. A "continuous" measuremem, for flow monitoring requirements, is a measurement obtained Gom an automatic recording device, <br />which continually measures flow. <br />2. A "grab" sample, for monitoring requirements, is a single "dip and take" sample. <br />3. An "in-situ" measurement, for monitoring requirements, is defined as a single reading, observation, or messtnement taken in the field <br />at the point of discharge. <br />4. An "instantaneous" measurement, for monitoring requirements, is a single reading, observation, or measurement performed on site. <br />
