PART I <br />Page le of 19 <br />Permit No. CO-0000132 <br />B. MONITORING REQUIREMENTS <br />2. Reporting of Data <br />Reporting of the data gathered in compliance with Part I.B.1 shall be <br />on a quarterly basis. <br />Monitoring results obtained during the previous 3 months shall be <br />summarized and reported on Division approved discharge monitoring <br />report (DMR) forms, postmarked no later than the 28th day of the month <br />following the completed calendar quarter. For example, for the 1st <br />quarter (January, February, and March) the quarterly report must be <br />received at this office postmarked no later than April 28. If no <br />discharge occurs during the reporting period, "No Discharge" shall be <br />reported. <br />Duplicate signed copies of the above report forms shall be submitted <br />to the following addresses: <br />Colorado Department of Health <br />Water Quality Control Di•ri sion <br />4210 East 11th Avenue <br />Denver, Colorado 80220 <br />Attention: Permits and Enforcement <br />U.S. Environmental Protection Agency <br />Denver Place <br />999 18th Street, Suite 500 <br />Denver, CO 80202-2405 <br />Attention: Water Management Division <br />Compliance Branch 8WM-C <br />3. Storm Exemption - Discharge points 003, 004, 005, 006, 007, 009 and 010 <br />If the permittee intends to use the 10-year, 24-hour storm exemption, <br />a report must be submitted within 90 days after the effective date of <br />this permit, documenting that the facility is designed, constructed <br />and will be maintained to contain or treat: <br />(a) The maximum volume of wastewater which would be generated by the <br />facility during a 24-hour period without an increase in volume <br />from precipitation; and <br />(b) The maximum volume of wastewater resulting from a 10-year, <br />24-hour precipitation event, including the volume which would <br />result from all areas contributing runoff to the individual <br />treatment facility (i.e., all runoff not diverted from the active <br />mining area or the mill area). <br />
