Cattany <br />Division Director <br />Natural Resourtt Trustee <br />On January 30, 2004, the Division of Minerals and Geology received your the 110(2) Hard ]tock/Metal Mining <br />Reclamation Pemut Application package for the The Mountain Mining Claim, File No. M-2004-008. <br />Preliminary review of the information received determined that the following items must be received before the <br />Division can consider your application as being submitted and technical review can begin: <br />APPi.if'ATinN FnRM~ The Division of Minerals and Geology did not receive a copy of the application <br />form. Please submit a copy of the n; ,nlirnt;nn form plus a copy of 911 P~+hit~ and two copies of the <br />missing exhibits listed below. <br />F.XHiRiT R _ Cite 1k•arrintinn nle, 6.3 2(Tll~ you must address Item (a) to the extent necessary to <br />demonstrate compliance with the applicable performance standazd requirements of Rule 3. At a <br />minimum, you must include a description of the vegetation and soil charactersstics in the azea of the <br />proposed operation. The local office of the Natural Resources Conservation Service (MRCS) may <br />pmvide you with this information as well as recommendations for Exhibit D -Reclamation Plan; <br />r'.XittRiT P _ RerlaTnntinn Pl9n (Role 6.i_41- (1) The purpose Of the lteclamatiOn Plan lS to describe <br />the timing, procedures, criteria and materials that will be used to reclaim the affected land to the proposed <br />future land use. This plan must be correlated to Exhibit E -Map. The description of the Reclamation <br />Plan must be adequate to satisfy the requirements of Section 3.1 and demonstrate compliance with <br />Rule 3. At a minimum, the Application shall include the following information; <br />(2) All 110 or 110d Limited Impact application must provide an estimate of the actual costs <br />to reclaim the site based on what it would cost the State of Colorado using an independent contractor to <br />complete reclamation. (Such estimates are not required for activities contemplated by the operator and <br />approved by the Office to be outside the scope of the proposed reclamation plan.) The unite costs should <br />include estimates for the following activities as appropriate to the operation: bacldilling; grading, topsoil <br />application, seeding, mulching, fertilization, and labor to complete rec]amation. Determine and specify <br />the poi during the operation when the site has reached a point of maxirnum disturbance. The cost to <br />reclaim the site to the specifications of the Reclamation Plan at this pointmust be estimated. Unit costs <br />(cost per cubic yazd), volumes, haul or push distances, and grades must be included when backfilling and <br />grading are part of the Reclamation Plan. Volume and unit costs for finish grading, subsoil and topsoil <br />application must be provided in terms of cost per cubic yard.
