Preliminary review of the informafion received <br />determined that the following items must be received before the Division can consider your application as being <br />submitted and technical review can begin: <br />F.XHiRiT R _ Site Descr~ntinn (Role 63.2l~1L• You must address Item (a) to the extent necessary to <br />demonstrate compliance with the applicable performance standazd requirements of Rule 3. At a minimum, you <br />must include a description of the soil characteristics in the azea of the proposed operation. The local office of <br />the Natural Resources Conservation Service (NRCS) may provide you with this information as well as <br />recommendations for Exhibit D -Reclamation Plan; <br />F.XRiRiT H - Mnniclpalities Within a Twn-Mile Radios [Rule 63.R1e List the mailing address and telephone <br />number of the governing body for all municipalities within a 2-mile radius of the proposed mining operation. <br />F.XNFRFT i. - Aermanent Man_Made Strnchrres [Role 6.i_121e You must provide information sufficient to <br />demonstrate that the stability of any structures located within two hundred (200) feet of the operation or affected <br />land will not be adversely affected.
