In the interest of efficien processing, <br />please check the appropriate box to indicate which type of application ou are applying <br />for. 111 permits may be issued only under special circumstances. Be su re you are <br />eligible. <br />LIh1ITED IMPACT OPERATIOIJS must disturb less than ten acres (i luding all <br />roads, stockpile areas and any other surface facilities) over the life of the <br />mine, and must extract LESS THAN 70,000 tons of mineral, over urden or <br />combination thereof per calendar year. If the affected area i s some fraction <br />greater than nine acres, be sure NOT io round this figure up t ten acres in <br />the narratives or elsewhere. <br />SPECIAL (ten-day Drocessing) operations are any sand, gravel, r quarry <br />aggregates operations which are to be operated for the sole pu use of <br />obtaining materials for Ylighway, road, utility, or similar-typ construction <br />under a Federal, State, county, city, town, or special distric contract where <br />the contract calls for work to be commenced within a specifica lly short time <br />and which will affect ten acres or less (including all roads, tockpile areas, . <br />and any other surface facilities). , <br />INSTRUCTION: This application and all accompanying materials as outline below must be <br />submitted in duplicate.
