Siznacorv Requirements <br />A11 reports required for submittal shaii be signed and certified for <br />accuracy by the permittee in accord ~aich the following criteria: <br />a) in the case of corporations, by a principal executive officer of <br />at least the Level of vice-president or his or her duly <br />authorized representative, if such representative is responsible <br />foc the overall operation of the facility from which the <br />discharge described in the form originates; <br />b) In the case of a partnership, by a general partner; <br />c) In the case of a sole proprietorship, by the proprietor; <br />d) In the case of a municipal, state, or other public facility, by <br />either a principal executive officer, ranking elected official, <br />or other duly authorized employee. <br />PART II <br />A. MANAGEMENT PEQUIREMENTS <br />1. Chan,¢e in Discharge <br />The permittee shall inform the Division (Permits and Enforcement <br />Section) in writing of any intent co cons[ruct, install, or alter any <br />process, facility, or activity [hat is likely to result in a new or <br />altered discharge, and shall furnish the Division such plans and <br />specifications which the Division deems reasonably necessary co <br />evaluate the effect on the discharge and receiving stream. <br />The permittee shall submit this notice within two (2) •aeeks after <br />making a determination co perform the type of activity referred to in <br />the preceding paragraph. Process modifications include, but are not <br />Limited co, the introduction of any new pollutant hoc previously <br />identified in the permit, or any ocher modifications 'which may resuit <br />in a discharge of a quantior or qualit•r different from chat which ': as <br />evaluated in the drafting of the permit including subsenuenc <br />amendments. Following such notice, the permittee shall be required to <br />submit a new GDPS application and this application may be processed as <br />an application for an individual permit if the application shows chat <br />the new discharge is ineligible for coverage under the general <br />permit. In no case shall the permittee implement such change •aithout <br />first notifying the Division. <br />
