Box 176, Oak Creek, CO 80467 <br />(970)736-8111 <br />Colorado Department of Health January 2, 1996 <br />WQCD-PE-B2 <br />4300 Cherry Creek Drive South <br />Denver, CO 80222-1530 <br />Dear Sir/Madam: <br />This letter will serve as follow up, and conclusion, to an incident <br />of toxicity reported from site DMR 006 at the Edna Mine site. <br />The site failure on the ceriodaphnia test species was reported to <br />the Colorado Department of Health, via phone conversation with <br />Kathleen Sullivan, on October 30, 1995. This notice was followed <br />with a written notice as part of the mine's regular fourth quarter <br />reporting. <br />The sampling for the accelerated test at the site was conducted <br />during the weeks of 11/13/95 and 11/27/95. Both sets of <br />accelerated testing passed the criteria for lethality outlined in <br />the mine's discharge permit.
