Reportine to Municioality <br />Permit No. CQG-500000 <br />Any permined facility dischazging to a municipal storm sewer shall provide the municipality with a copy of the pemrit application, <br />upon request. A copy of the SWMP shall also be provided to the municipality upon request. <br />8. Special Monitorine <br />a. TDS monitoring is only required for facilities dischazging in the Colorado River Basin. TDS may be measured as electrical <br />conductivity where a satisfactory correlation with TDS has been established based upon a m;nimt,m of five {5) samples. See <br />the individual Certification Rationale for a determination as to whether monitoring is required. <br />b. <br />Phosphorus monitoring is required only for facilities which discbazge into the following drainage basins: Cherry Creek <br />basin, Chatfield Reservair downstream of the USGS gage at W aterton and on Plum Creek, Dillon Reservoir basin (i.e., Ten <br />Mile Creek, Snake River, Blue River, all tributaries to the Dillon Reservoir), and Bear Creek basin. See the individual <br />Certification Rationale for a determination as to whether monitoring is required. <br />9. SienatorvReuuuements <br />a. All reports required for submittal shall be signed and certified for accuracy by the pemrittee in accordance with the following I <br />criteria: <br />]) <br />2) <br />3) <br />4) <br />In the case of corporations, by a principal executive officer of at least the level ofvice-president or his or her duly <br />authorized representative, if such representative is responsible for the overall operation of the facility from which the <br />discharge desrnbed in the form originates; <br />In the case of a partnership, by a general partner; <br />In the case of a sole proprietorship, by the proprietor, <br />In the case of a municipal, state, or other public facility, by either a principal executive officer, ranking elected <br />official, or other duly authorized employee. <br />b. Changes to authorization. If an authorization under pazagraph a) of this section is no longer accurate because a diffe <br />individual or position has responsibility for die overall operation of the facility, a new authorization satisfying <br />requirements of paragraph a) of this section [Host be submitted to the Division, prior to or together with any reps <br />information, or app&cadons to be signed by an authorized representative. <br />c. Cettif cation. Any person signing a document under paragraph a) of this section (except for the SWMP certification) <br />make the following certification: <br />"I certify under penalty of law that this document and all attachments were prepazed under my direction or supervision <br />accordance with a system designed to assure that qualified personnel properly gather and evaluate the information submit <br />Based on my inquiry of the person or persons who manage the system, or those persons directly responsible for gathering <br />information, the information submitted is, to the best of my knowledge and belief, tme, accurate, and complete. A "continuous" measurement, for flow monitoring requirements, is a measurement obtained from an automatic recording de <br />which continually measures flow. <br />2.
