Listed below aze the Division's adequacy <br />review comments for this application. The Division is required to make an approval or denial decision no later than May 8, <br />2002 therefore, your response to the following adequacy review concerns should be received by the Division by Ma3,2002. <br />1. As required by Rule 1.6.5(2) please submit the proof of publication of the public notice circulated in the local news <br />paper and all owners of record of surface lands, and easement holders that are on the affected landand within 200 feet <br />of the boundary of the affected land. <br />2. Map Exhibit E has a surveyed boundary which appeazs to be the proposed permit boundazy, but which is not labeled as <br />such. Please submit an Exhibit E Map with a marked and labeled permit boundary. - <br />Pleased be advised that the Quinn Draw Gravel Pit 110 (c }Application may be deemed inadequate, and the application <br />may be denied on May 8, 2002 unless the above mentioned adequacy review items aze addressed to the satisfaction of the <br />Division. If you feel that more time is needed to complete your reply, the Division can grant an extension to the decision <br />date. This would be done upon receipt of a written waiver of your right [o a decision by May 8, 2002 and a request for <br />additional time. This must be received no later than the deadline date.
