<br />~~ <br />MAPS AND EXHIBITS <br /> <br />With each copy of the application form, the applicant must submit a corres- <br />pondi~g set of the maps and exhibits outlined in Rule 3 and 6 as follows: <br />1. EXHIBIT A - The address of the gravel office and the local address <br />of the operator. <br />~. EXHIBIT B - The name of the owner of the surface of [he affected <br />land. <br />:S. EXHIBIT C - The name of the owner of the subsurface rights of the <br />affected land. <br />4. EXl1IBIT D - A map showing information sufficient to determine the <br />location of the affected land on the ground and exis- <br />ting and proposed roads or access routes to be used <br />in connection with the operation. <br />_. EXHIBIT E - Legal description (to the nearest quarter, quarter <br /> section) and approximate size of affected land. <br />E. EXHIBIT F - Mining plan. <br />7. EXHIBIT C - Reclamation plan. <br />8. EXHIBIT H - List of all permits and approvals from the County. <br />9. EXHIBIT I - Copies of lease and deeds <br /> Deed - EXHIBIT -1 - 1 <br /> Lease Agreement - EXHIBIT - I - 2 <br />10. EXHIBIT J - Proof of publication and notice <br />11. EXHIBIT K - Soil and Vegatation information. <br />12. EXHIBIT L - Reclamation costs. <br />13. EXHIBIT M - Plat of survey. <br />
