Dept. of Natural Resources <br />OPEN PIT GRAVEL MItJING OPERATION <br />Dear Mr. O'Connor: <br />Enclosed are the following items that you have requested to complete this appli- <br />cation for presentation to the Board, <br />A check for $68.00 to cover all the acreage within the Pemit area, <br />Four (4) sets of Revised Maps (Sheets 2 & 3) deleting previous Area 3 to <br />correct the proposed Mining and Reclamation Plans. <br />Four (4) sets of the following report pages revised to the final proposal: <br />D-1; D-4; D-5; D-6; and E-1, <br />V Four (4) copies of map showing adjacent owners. <br />Four (4) copies of the unilateral agreement with owners of improvements <br />within 200 feet. (Same as submitted by Robert C. Bosley, 11-2-83). <br />Summary of Reclamation Costs and Band amounts are outlined on Pages <br />L-1 and L-2. <br />Copies of Certified Mailings to each easement holder. <br />~~ Four (4) copies of Weld County Commissioners'Resolution on October 5, 1983 <br />granting local approval, (Pages Q2 - 1 through Q2-4). <br />~> Four (4) copies of "Second Addendum to Sand & Gravel Lease between La <br />Poudre Sand & Gravel and Loveland Excavation to clarify access and <br />possession rights.
