August 18, 2004 ..................... <br />~vironmental Services <br />a division of Jericho Industries <br />Tom Bird <br />Environmental Chemistry National King Coal <br />4424 CR 129 <br />Hesperus, CO 81326 <br />Site Sampling 8 Testing RE: Missing reports <br />Dear Tom, <br />Site Assessments Asper our recent telephone discussion CDS has determined that the reports <br />for the fourth quarter of 2003, the first quarter 2004 and the current report for second <br />quarter 2004 were not sent. Examination of our files determined that the reports and <br />their invoices were processed, printed and signed.. At this time the files were filed <br />without your original copy mailed. This mishandling of data was seen for severs! <br />Strategic Planning clients within this time period. CDS attributes this to a former part time employee that <br />was not paying attention. The packages were signed and ready to send, but were <br />filed along with the CD5 file copy. <br />If National King Coal requires additional documentation or letters sent to <br />Soil & Geochemistry regulators these will be provided at no cost. CDS has discounted the report from <br />~ fourth quarter 2003 due to it's lateness. <br />On another matter CDS has scheduled the sampling of your sites to occur <br />during the middle month of each quarter to void the sampling problem seen last <br />Safety assessments quarter.
