Enclosed is a form which need to be completed by you as legal contact prior to the <br />Division terminating the coverage of this site under the general permit. <br />Please complete the enclosed inactivation form. If an area is not applicable, please so <br />indicate. Based on the information you have provided to the Division tl7usfar, we anticipate <br />that we can inactivate your entire permit. <br />Upon receipt of the completed form, the District Engineer for the above-referenced county <br />will be notified and will inspect your facility. Regulations require that monitoriL-g reports <br />be submitted until the permit is formally terminated.
