PART II <br />Page 14 <br />PERMIT NO.: COG-500000 <br />A. MANAGEMENT REQDTRx~rRa'r$ (CONTINOED) <br />14. Signatory Requirement <br />All reports required for submittal shall be signed and certified for <br />accuracy by the permlttee in accord with the following criteria: <br />a) In the case of corporations, by a principal a:ecuCive officer of <br />at least the level of vice-president or his or her duly authorized <br />representative, if such representative is responsible for the . <br />overall operation of the facility from which the discharge <br />described in the form originates; <br />b) In the case of a partnership, by a general partner; <br />c) .In the case of a sole proprietorship, by the proprietor; <br />d) In the case of a municipal, state, or other public facility, by <br />either a principal ezecutive officer, ranking elected official, or <br />other duly authorized employee. <br />Within sizty (60) days of certification the permittee shall file a <br />statement with the Environmental Protection Agency and the Water <br />Quality Control Division which shall contain the names of the <br />person or persons who are designated to report conditions on <br />discharge monitoring reports and as noted in "Noncompliance <br />Notification", Part II A.3. The permittee shall continually <br />update this list as changes occur at the facility. <br />B. RESPONSIBILITIES <br />1. Inspections and Right to Entry <br />The permittee shall allow the Director of the State Water Quality <br />Control Division, the EPA Regional Administrator, and/or their <br />authorized representative, upon the presentation of credentials: <br />a) To enter upon the permittee's premises where a regulated facility <br />or activity is located or in which any records are required to be <br />kept under the terms and conditions of this permit; <br />b) At reasonable times to have access to and copy any records <br />required to be kept under the terms and conditions of this permit <br />and to inspect nay monitoring equipment or monitoring method <br />required in the permit; and <br />Code: i - 29 Date: 1 - S4 <br />
