- --
-5-
COLORADO WATER LAWS

The permittee is encouraged to contact the State Engineer, with regard to the
administration of water rights, and the Colorado Department of Health, with regard to
the discharge of pollutants into the waters of the State. Violation of Colorado water
laws could result in enforcement actions taken by the State Engineer and/or the Colorado
Department of Health.
ADDITIONAL INFORMATION
This space can be referenced for further information of any of the items on this form.
It should be noted that there was an open pit mining operation in the same
general area several years ago (shown as a "gravel pit" on the 1970 topographic
map at EXHIBIT A). Mitchell Concrete & Caisson Drilling, Inc does not intend
to reclaim that part of the former operation that is not used under this permit.
To the best of my knowledge, all the information presented in this application is true
and correct.
Date: February 27, 1986
~-
il~-o
T' e
(Rev. 7/84)
Doc.
