o • <br />bottom of the casing. American Soda will establish a <br />cement plug from above the CIBP to the surface. If it <br />is not possible to run a CIBP inside the intermediate <br />casing due to well bore irregularities, a cement basket <br />shall be run in the hole to establish the cement plug <br />at the bottom of the intermediate casing. <br />Both the surface and intermediate casings shall be cut <br />down to 1 to 3 feet below ground level to salvage the <br />wellhead. Cutting down the casings will also allow for <br />more complete surface reclamation, which was requested <br />by the BLM. <br />The cement plug shall be set using one or more of the <br />approved methods described in 40 CFR § 146.10. The <br />number of sacks used for the cement plug shall be <br />calculated based on available casing diameter <br />information such that the resulting cement plug will <br />extend vertically for the required vertical length <br />inside the intermediate casing. A dry hole marker <br />shall be installed which will have the name of the <br />operator, well number, lease number and the legal <br />description welded on the surface. Surface facilities <br />associated with the various wells shall be removed, and <br />the well pad(s) shall be reclaimed according to BLM <br />specifications. <br />3. Pluaaina and Abandonment Report. Within sixty (60) <br />days after plugging the well, the Permittee shall <br />submit a report on Form 7520-13 to the Director. The <br />report shall be certified as accurate by the person who <br />performed the plugging operation and the report shall <br />consist of either: (1) a statement that the well was <br />plugged in accordance with the plan; or (2) where <br />actual plugging differed from the plan, a statement <br />that specifies the different procedures followed. <br />(d) Modification of Pluaaina and Abandonment Plan. If a <br />well or cavity failure occurs that may affect USDWs, <br />the Permittee is required to modify the plugging and <br />abandonment plan to address any existing or future <br />problems as required by 90 CFR § 146.10 (d). The <br />Director will prescribe aquifer clean up and monitoring <br />or other measures where necessary to ensure adequate <br />protection of overlying USDWs as needed. <br />EPA FINAL Area Permit No. CO3858-00000 <br />Page 28 of 164 <br />
