The application for a reclamation permit for this operation was deemed complete and filed for technical review on
June 18, 2002. Before a decision can be made on your application the following items need clarification. Please
provide a response to each item prior [o September 16, 2002. If you are unable to fully respond [o this letter, please
contact me requesting continuance of consideration [o a later date.
Please be advised that your application may be deemed inadequate and denied on September 16, 2002 unless [he
following adequacy review items aze addressed to the satisfaction of the Division.
EXHIBIT C -Pre Mining and Mining Plan Map/s of Affected Lands
Rule 6.4.3(a) requires that this map show all adjoining surface owners of record. If Ronnie Davis owns all
the land adjoining the proposed permit area, please show this on the Exhibit C Map. If other owners have
land adjoining the proposed permit area show them on the map.
2. Is there is any land within the proposed permit boundary that will be excluded form mining activities? If so,
please show this by marking an "Affected Land Boundary" on the map. Is [he 200 fee[ setback from all
permanent man-made structures such as the pond dam spillway within the permit boundary? If so, show it on
the map.

3. In accordance with Rule 6.4.5(2)(f)(i), state the maximum anticipated final slope gradient.

4. Show on this map [he proposed topography of [he area with comour lines of sufficient detail to portray the
direction and rate of slope of all reclaimed lands.

5. The Division of Wildlife Statement included as your Exhibit H recommends that all mining and reclamation
activities should maintain a minimum setback of 200 feet from the edge of the pond. Please commit [o such a
setback from the high water line of the pond and if necessary show it on your Exhibit C Mining Plan Map.
