I <br />1 -5- <br />EXHI6ITS A-J <br />Two (2) complete application packages must be submitted. One complete application <br />package consists of the application form and the set of maps and exhibits referenced <br />I below as Exhbits A-J. Each exhibit must be presented as a separate section. Begin each <br />exhibit on a new page. Pages should be numbered consecutively for Base of reference. <br />If separate bound documents are used as appendices, please reference these by name in <br />I the exhibit. <br />EXHIBIT A - Leaal Description and Location Map <br />I The legal description must identify the affected land, specify affected areas and be <br />adequate to field locate the property. Description shall be by township, range, and <br />section, to at least the nearest quarter-quarter section. A metes and bounds survey <br />I description is also acceptable. Also, the street address or lot number(s) may be used. <br />This information 1s available from the County Assessor's office or U.S. Geological <br />Survey (USGS) maps. NOTE: If you improperly locate your operation 1n the field, and as <br />a result mine outside the area delineated by this permit application, 1t could be <br />I considered to be a violation of 34-32-123 or 34-32-124, C.R.S. of the Mined Land <br />Reclamation Act. <br /> A map showing information sufficient to determine the location of the affected land on <br />I the ground and existing and proposed roads or access routes to be use d in connection <br /> with the mining operation. Names of all immediately adjacent surface owners of record <br />I shall also be shown. The operation <br />Geological Survey map. The location location map shall be a st <br />of the proposed operation andard <br />shall 1:24,000 scale U.S. <br />be shown and <br /> labeled with the mine site name. <br />I EXHIBIT B - Site Oescription <br />Items 1-3 below must be addressed to the extent necessary to demonstrate compliance with <br />I the applicable performance standard requirements of Rule 6. At a minimum, you should <br />include the following information: <br />I 1. A description of the vegetation and soil characteristics in the area of the <br />proposed operation. The local office of the Soil Conservation Service (SCS) may <br />provide you with this information as well as recommendations for Exhibit D - <br />Reclamation Plan. Contact the Regional SCS office at 655 Parfet Rm E2000, <br />I Lakewood, Colorado 80215, (303) 236-2886 or the Mined Land Reclamation Division <br />for assistance in locating the office nearest you; <br />I 2. Identify any permanent man-made <br />area and the owner of each structure. structures within 200 feet of <br />Permanent man-made structures the affected <br />include <br /> houses, barns, fences, above or below ground utilities, irrigation d itches, <br /> maintained or public roads, bridges, wells, pipelines, water storage impoundments, <br />I railroad tracks, cemeteries, and communication antennas, etc. Each structure <br /> should be located on Exhibit E - Map. <br />I 3. A description of the water resources in the area of the proposed operation. <br />Identify any streams, springs, lakes, stockwater ponds, ditches, reservoirs, and <br />aquifers which would receive drainage directly from the affected area. Provide any <br />' information available from publications or monitoring data on flow rates, water <br />table elevations and water quaiity conditions. <br />1 <br />
