A letter of certification <br />containing the results of the inspection must be submitted to the <br />Division within two weeks following the inspection." <br />In accordance with this stipulation, I, Michael G. Altavilla, <br />Colorado P.E. 28455, inspected the subject embankment on June 30, <br />1999 and do herewith submit my report of findings: <br />I. Chronology <br />A. July 14, 1981: F.M. Fox and Associates submitted a report <br />on their geotechnical slope stability investigation of <br />the embankment. The report concluded that the embankment <br />was stable with a minimum static safety factor of 1.52. <br />The report also recommended revegetation and inspections. <br />B. September 23, 1981: Colorado Mining Permit No. C-80-005 <br />became effective. <br />C. Fall 1981: The embankment was hand seeded in compliance <br />with Stipulation 16 of Permit No. C-80-005. <br />D. June 15, 1982: The annual embankment inspection was <br />conducted. Weather conditions were favorable, skies <br />partly cloudy, temperature about 65° F. A light rain had <br />fallen on the evening of June 14th. <br />E. July 18, 1983: The annual embankment inspection was <br />conducted. Weather conditions were favorable, skies <br />partly cloudy, temperature about 84° F.
