FOChI1P3Cl <br />E+ecutire Director <br />Michael 9. long <br />Dig i>ion Dheaor <br />On 3/13/98, DMG received a 110 Construction Materials Reclamation Permit Application <br />package for the Conger Harvesting Area, Permit No. M-98-022. Preliminary review of the <br />information received determined that the following items must be received before DMG can <br />consider the application as being submitted and technical review can begin: <br />1. Application Form: Corporate seal and signature of the corporate secretary was <br />missing on page 6 of the application form. The original application, page 6, is being <br />returned from the Denver office for embossment of the corporate seal and signature of the <br />corporate secretary. If a corporate seal does not exist, please indicate at the location <br />available for the seal. If a corporate secretary does not exist, please indicate at the <br />location available for the secretaries signature. <br />2. Exhibit B -Site Description, Rule 6.3.21b): Please ensure that all permanent man- <br />made structures within 200' of the affected area (permit boundaryl, and owners of each <br />structure, are identified. Additionally, each structure must be located on Exhibit E, Map. <br />Please clarify whether the 2 small lakes, identified within Exhibit B are within 200', <br />and if so, please show their locations on Exhibit E, Map. <br />Please identify the overhead powerline and its owner. <br />Please identify County Road 3 (CR 3) and its owner. <br />Please identify the land owner for the land shown on Exhibit E, Map, as "commercial <br />parcel 1.27 acres". <br />Please identify the existing access road, located within 200' of the permit boundary, <br />and identify the owner of all portions within 200' of the permit boundary. <br />
