<br />Rule 6.4.13 EXHIBIT M -Other Permits and Licenses <br />Colorado State Historical Society. Based upon our previous experience with the Colorado State <br />Historic Preservation Office regazding clearances for mine pits, we have been informed they will not <br />send us written information regarding this matter and that consultation in writing will only be made <br />with the Colorado Division of Minerals and Geology. <br />Colorado Division of Water Resources. Consultation with the Colorado Division of Water Resources <br />indicates that a well permit will be required to proceed with this gravel mining operations since <br />ground water will be intercepted by the proposed mining operation. A Gravel Pit Well Permit <br />Application form has been filed with the Office of the State Engineer. <br />Colorado Department of Health, Water Oualit~Control Division. Copies of the permit application <br />forms for the NPDES and Storm Water Dischazge Permits have been filed with the Colorado <br />Department of Health. <br />Colorado Department of Health, Air Pollution Control Division. The Applicant has field a permit <br />application form for the Dus[ Emissions Notice with the Colorado Department of Health, Air <br />Pollution Control Division. The Applicant was informed in a letter dated ??? that this permit has <br />been approved.. <br />U. S. Army Corps of En ineers. Section 404 of the Clean Water Act requires that all activities which <br />result in the placement of fill materials or excavation of all waters of the United States will trigger <br />a 404 Permit. Jurisdictional waters of the United States in the form of wetlands exist in the vicinity <br />of the proposed Camilletti Milner Pit # 2 Gravel Pit. In order to address this issue a formal wetland <br />delineation was conducted on this property in the fall of 1999. <br />The results of this investigation were presented to Mrs. Sue Nal] of the Corps of Engineers and she <br />conducted a site visit to the azea on 7 May 1997 and she verbally acknowledged that the map the <br />Routt County 50 foot stream set back and accompanying hillside wetland area, shown on Map 2, <br />Camilletti Milner Pit # 2 -Existing Conditions Map, shows the location of the all of the wetlands <br />found in the vicinity of the mine azea. The site visit to the property revealed that the only wetland <br />impact associated with this proposed action will be for a road crossing of the drainage and a minor <br />amount of wetland excavation. Enclosed aze letters from the US Army Corps of Engineers <br />documenting that this project will not require a 404 Permit. <br />Routt County. The Routt County Master Plan was amended on February 4, 1992 and requires that <br />all sand and gravel mining operations be permitted via a Special Use Permit. Pursuant to these <br />regulations, a Special Use Permit Application .vas filed with the Routt County Regional Planning <br />Department on 13 Apri12000 [o operate this gravel pit and haul the aggregate on Routt County Road <br />Number 179. Routt County has approved the Conceptual SUP and approval was obtained from the <br />Routt County Regional Planning Commission for the Final Special Use Permit relative to this project <br />on 1 Mazch 2001. The final public hearing relative to this project is scheduled before the Routt <br />26 <br />
