APPLICATION FORM: I have provided as updated form for you to resubmit. Please be sure to submit one (I)
completed, signed and notarized ivory original and one (I) copy of We completed ivory application form. Also, please
be sure that the corporate seal for Lyman-Ricky Corporation dba Central Sand & Gravel is affixed next to the signature
of the Corporate Secretary or Equivalent on page 5.
EXHIBIT J -Proof of Hotline Notices of Permit Appication (Rule 6.3.101: Include proof of tnailiogs of the 'Notice
of Permit Application" sent to the Boazd of Supervisors of the local Soil Conservation District, in the application, by
submitting the date-stamped receipt for Certified Mailing or adate-stamped copy of the notice form with signamre
acknowledging personal service and receip[ by the appropriate board. Also, refer to Rule 1.6.2(1)(a).
EXHIBIT K -Terms of Governmental Contrast (Rule 6.3.111: You must provide a copy of the signed government
contract or other documentation verifying the necessity of a Special Ill Operation permit (i.e. the contract be[ween

operator on the application. When returning the bond, please be sure to affix the cotporate seal for Lyman-Richey
Corporation dba Central Sand & Gravel next to the signature of Thomas Baughman.
