PART I <br />Page 2 <br />Permit No. COG-850000 <br />• <br />A. COVERAGE DNDER THIS PERMIT <br />1. This general permit authorizes discharges that consist entirely of <br />precipitation induced surface runoff from coal mining anti processing <br />operations and loadouts, while active mining ie taking place or during <br />reclamation, into waters of the State of Colorado. E=ceptiona are <br />those facilities in the Cherry Creek Basin upstream of Cherry Creek <br />Reservoir and those tributary to Chatfield Reservoir doin:etream of the <br />DSGS gage at Waterton or is the Plum Creek drainage. No mine water, <br />preparation plant water, spoils spring water, domestic sewage or other <br />wastewater other than surface runoff may be discharged. Multiple <br />discharges from one facility maq be authorized through tY:ia permit but <br />ALL DISCHARGES FROM A FACII.TY COVERED BY THIS PERMIT MDS7' CONSIST <br />ENTIRELY OF SIIRPACE RIINOFF. If a facility has discharger from one or <br />' more of the above ezcluded sources alone or in addition t:o surface <br />runoff discharges, an individual permit is required. <br />2. Ia order to apply for certification under this general peermit, the <br />owner, operator, sad/or authorized agent of the subject facility shall <br />submit by certified mail or hand delivery, the completed Coal Mining <br />Wastewater Discharge Application (furnished by the-division) which <br />generally requires the following information (at a minimum): <br />• (a) Name, address and descriptive location of the facility; <br />(b) Name of principal in charge of operations; <br />(c) Name of the water receiving the discharge; <br />(d) Description of the activity including the anticipated duration of <br />activity and/or discharge, anticipated volume, and rate of <br />discharge; <br />(e) Description of any waste water containment and/or treatment; <br />(f) A map showing the general geographical location of t:he facility <br />and a sketch of the facility showing outfall(s) and receiving <br />water(s); and <br />(g) An analysis of the water to be discharged. <br />At least thirty (30) days prior to the anticipated date of discharge, <br />such information shall be submitted to: <br />Colorado Department of Health <br />Water Quality Control Division <br />4210 East 11th Avenue <br />- Denver, Colorado 80220 <br />"' Attention: Permits and Enforcement <br />• <br />
