Subsission of Incorrect or Incomplete Information <br />Wham the permittee failed to submit nay relevant facts is a permit <br />application, or submitted incorrect information is a permit <br />application or report to the Division, the permittee shall promptly <br />submit the relevant'application information xhich xas not submitted <br />or nay additional information needed to correct nay erroneous <br />information pravioualy submitted. <br />S. Bypass <br />The permittee may allox nay bypass to occur which does not cause <br />affluent limitations to be ezceeded, but if and only if it is for <br />essential maintenance to assure efficient operation. <br />Bypass ie prohibited, and the Division may take enforcement action <br />against a permittee for bypass, unless: <br />a) Bypass xas unavoidable to prevent loan of Life, personal <br />injury, or severe property damage; <br />b) There were no feasible alternatives to the bypass, such as the <br />use of auziliary treatment facilities, retention of untreated <br />wastes, or maintenance during normal periods of equipment <br />doxntima. Thin condition is not satisfied if the permittee <br />could have Snstalled adequate backup equipment to prevent a ' <br />bypass xhich occurred during normal periods of equipment <br />dowtime or preventative maintenance; sad <br />c) The permittee submitted notices as required in "Bypass <br />Notification", Part II.A.6. <br />6. Bypass Notification <br />If the permittee knows is advance of the need for a bgpas3, a notice <br />shall be submitted, at least tea days before the date of the bypass, <br />to the Division sad the Environmental Protection Agency (EPA).
