Midrefle L. Austtn November 25, 2005 ~~ 0742. The applicant shall maintain daily records of aN diversions, replaoemertts, and Uta amount of water used for each particular purpose. The applicant shall provide a report of these records to the division engineer and the water cwnmissioner on a mouthy basis on a form approved by them. The arxbunting must be submitted within fore (5j calendar days of the end of the month for which the accrounting ~ being made. in addition, the applicant shall submit a report from each entity making replacements; for the plan, those entities are the City of Greeley, Consolidated Mutual Water Company and the Saint Vrair- Left Hand Water Conservancy Disirid. The report shall include an accounting of all wastewater plant water effluent controlled by each entity, showing the total vdume of water under Hs control at the variars wastewater treatrrrent plarris, and the amount committed to each of the radpients of the water. 8. Applbant shall follow the accounting and recharge protocols as referenced in the attached documents for the operation of this SWSP. 9. /W releases of replacement water must be sufflder-t bo cover aU out ~ priority depletions and be made under the direction and/or approval of the Water Commissioner(s). The replacement may be aggregated to maximize benefidai use. The Water Comrtissbner(s) and/or DiNsfon Engineer shall determine the rate and timing of an aggregated release. 10. The replacement water, which is the subject of this plan cannot be sob or leased to any other entity. AS a rxxrditian of subsequent rerrewals of this sutxrtltute water supply Plan, the replacement water must be appurtenant to this sibs until a plan for augmentation Is obtained. A copy of thk approval letter should be recorded with the county dark and recorder. All replacement water must be noncurrent with depletions in quantity, timing, and locations. The use of replacement water must be in compliarrce with all agreements with the providers of replacement water. 11. Dewatering at the pits will produce delayed depletions ib the stream system. As bog as the pit to continuously dewatered, the weber returned to the stream system should txs adequate to offset the depletions. However, once dewatering at the site ceases, the delayed depletions must be addressed. At least three years prior to completion of mirdng, a plan must be submitted that specifies how the post pumping dewatering depletfona will be replaced in time, ~' place and amount. 12. Approval of this plan is for the purposes as stated herein. This otfioe must first approve any additional uses for which the water may be used. 13. The approval of this substHute water suppy plan does oat relieve the Applignt and/or landowner of the requirement to obtain a Water Court decree approving a permanent plan for augmentation or mitigation to ensure the pennarrent replacement of all depleflorrs, including long-term evaporation losses and lagged depletions after gravel mining operations have ceased. tf reclamation of the mine sites wfll produce a permanent water surface exposing groundwater to evaporetion, an application for a plan for
