This permit Is Issued in reliance upon the atourecy and Completeness of information supplied by the applicant and <br />is cdnditloned upon conduct Of the activity, or Construction, intallatlon and operetlon d the source, in accordance <br />with this informatOn and with representation made by U1e applipnt ar applipnhs agents. Section 25-7.114.7(2xa), C.R.S. requires that all sources required to file an Air Pollution Emission Notice (APEN) <br />must pay an annual tae to cover the ousts of irepectipns and administration. If a source or activity Is to ha <br />discominued, the owner must notify the DIVIsIOn in writing requesling a pncellatlcn of the Parton. Upon notlfidtion. <br />annum fee blUing will terminate. <br />9. Violatlan of the lertn of a permit or of the provision of the Colorado Air Pollution Prevention and Conlyd Act or the <br />regulations of the AQCC may result in admtnislreUve, evil or criminal enforcement action4 under Sections 25-7-115 <br />(enforcement). -121 (njuncfions), -122 (civil penalties), -122.1 (criminal peretUes), C.RS. <br />45117(! ver 11!99 . <br />
