MMRR quarry, M-2004-067 <br />Response to Sept. 7 Adequacy Review <br />September 12, 2005 <br />Page 2 <br />8. The Division requests that the Applicant commit to evaluating the need for a filter fabric or liner <br />beneath any riprap installation, and install such filter fabric if needed. Please respond. <br />The Applicant will evaluate the need for filter fabric or a liner beneath riprap in <br />surface drainage channels at the MMRR Quarry. Filter fabric or liner will be <br />installed if needed. <br />C. The Division requests that the Applicant commit to providing an 'As-Built Certification" from a <br />registered professional engineer for all hydraulic structures installed at the MMRR Quarry, <br />including drainage or collection channels, sediment ponds, spillways, riprap pads, check dams, <br />culverts, etc. The certification should be submitted within 60 days of completion of all hydraulic <br />structures and verify that all such structures were properly installed according to the designs <br />provided in the mining and reclamation permit. Please respond. <br />Within 60 days of completed construction of hydraulic structures installed at the <br />MMRR Quarry, the Applicant will supply to the Division an As-Built Certification <br />from a registered professional engineer. The certification will verify that <br />structures were properly installed according to designs in the mining and <br />reclamation plans. <br />21. Blasting Plan -Review comments from Allen Sorenson will be forwarded to the Applicant as <br />soon as they become available. <br />By topic, Allen Sorenson's September 9 memo is addressed as follows: <br />Blasting Vibration Compliance <br />Please see the attached letter from Lyman Henn, Inc. <br />Pre-Blast Surveys <br />The applicant will conduct pre-blast surveys of potentially affected structures in <br />accordance with DMG Rules, with notice and requests to survey sent to all <br />affected structure owners at least 30 days prior to initiation of blasting. <br />Furthermore, the survey will be completed and submitted to the DMG prior to the <br />initiation of blasting. <br />Conduct of the structure survey will be based on appropriate established <br />practices, as cited in Mr. Sorenson's memo. The Office of Surface Mining, <br />Blasting Guidance Manual (1987) shall define appropriate methods for conduct of <br />the survey. <br />Controls for Flyrock and Dust <br />Please see the discussion below. The response to the Division's comments in <br />the September 7 adequacy review letter also incorporates discussion of the <br />relevant portions (on page 4) of Allen Sorenson's September 9 memo. <br />
