Reportine of Data <br />Reporting of the data gathered in compliance with Part I.B.1 shall be on a Quarterly basis. <br />Monitoring results shall be summarized for each calendar quarter and reported on Division approved discharge <br />monitoring report (DMR) forms postmarked oo later than the 28th day of the month following the end of the <br />quarter (for example, the DMR for the first calendar quarter must be postmatlced by April 28th). If no discharge <br />occurs during the reporting period, "No Discharge" shall be reported. <br />The first orieinal signed copy of each discharge monitoring report (DMR) shall be submitted to the Division at the <br />following address: <br />Colorado Department of Flealdt <br />Water Quality Control Division <br />Permits and Enforcement Section <br />WQCD-PE-B2 <br />4300 Chetry Creek Drive South <br />Denver, Colorado 80222.1530 <br />The first duplicate signed copy of each discharge monitoring report (DMR) shall be submitted to the following <br />agency: <br />U.S. Environmental Protection Agency <br />Water Management Division <br />NPDES Branch 8WM-C <br />999 18th Street, Suile 500 <br />Denver, CO 80202-2466 <br />3. Freauencv and Sample Tvpe - Acute WET Tesline <br />Quarterly test results shall be reported along with [he Discharge Monitoring Report (DMR) submitted for the end <br />of the reportipg calendar quarter (i.e., WET testing results for the calendar quarter ending March 31 shall be <br />reported with the DMR due April 28, with the remaining WET testing reports submitted with DMRs due each July <br />28, October 28 and January 28). The results shall be submitted on the Acute Toxicity Test report form, available <br />from the Division.
