Revorting of Data <br />Reporting of the data gathered in compliance with Parts I.B.1 and I.B.2 shall be on a <br />o t basis. Monitoring results shall be summarized for each month and reported on <br />Division approved discharge monitoring report forms postmarked no later than the 28th <br />day of Che following month. If no discharge occurs during the reporting period, "No <br />Discharge" shall be reported. <br />The first o inal copy of each discharge monitoring report (DMR) shall be submitted to <br />the Division at the following address: <br />Colorado Department of Health <br />WQCD-PE-B2 <br />4300 Cherry Creek Drive South <br />Denver, Colorado 80222-1530 <br />The first duvlicate signed copy of each report form shall be submitted to the following <br />agency: <br />U.S. Environmental Protection Agency <br />Water Management Division <br />NPDES Branch SWH-C <br />999 18th Street, Suite 500 <br />Denver, CO 80202-2466 <br />4. Chronic WET Testing-Outfalls 001. 005 and 007 <br />(a) Testine and Revorting Requirements <br />Tests shall be done at the frevuencv listed in Part I.B.1. Test results shall be <br />reported along vith the Discharge Monitoring Report (DMR) submitted for the <br />reporting period during which the sample vas taken. (i.e., WET testing results for <br />the first calendar quarter ending March 31 shall be reported with the DMR due April <br />28.) The results shall be submitted on the Chronic Toxicity Test report form, <br />available from the Division. Copies of these reports are .to be submitted to both <br />the Division and EPA along with the DMR. <br />The permittee shall conduct each chronic WET test in general accordance with <br />methods described Short Term Methods for Estimating the Chronic Toxicity of <br />Effluents and Receiving Waters to Freshwater Or anisms, EPA/600/4-89/001 or the <br />most current edition, except as modified by the most current Division guidance <br />document entitled Guidelines for Conductive Whole Effluent Toxicity Tests.
