Monitoring results shall be summarized for each month and reported on Division approved discharge monitoring report forms postmarked no later than the 28th day of Che following month. If no discharge occurs during the reporting period, "No Discharge" shall be reported. The first o inal copy of each discharge monitoring report (DMR) shall be submitted to the Division at the following address: Colorado Department of Health WQCD-PE-B2 4300 Cherry Creek Drive South Denver, Colorado 80222-1530 The first duvlicate signed copy of each report form shall be submitted to the following agency: U.S. Environmental Protection Agency Water Management Division NPDES Branch SWH-C 999 18th Street, Suite 500 Denver, CO 80202-2466 4. Chronic WET Testing-Outfalls 001. 005 and 007 (a) Testine and Revorting Requirements Tests shall be done at the frevuencv listed in Part I.B.1. Test results shall be reported along vith the Discharge Monitoring Report (DMR) submitted for the reporting period during which the sample vas taken. (i.e., WET testing results for the first calendar quarter ending March 31 shall be reported with the DMR due April 28.) The results shall be submitted on the Chronic Toxicity Test report form, available from the Division. Copies of these reports are .to be submitted to both the Division and EPA along with the DMR. The permittee shall conduct each chronic WET test in general accordance with methods described Short Term Methods for Estimating the Chronic Toxicity of Effluents and Receiving Waters to Freshwater Or anisms, EPA/600/4-89/001 or the most current edition, except as modified by the most current Division guidance document entitled Guidelines for Conductive Whole Effluent Toxicity Tests.
