DBMS is required to <br />issue an approval or denial decision no later than 2 January 2006 for this application. <br />However, per your discussion with Tony Waldron of our office, the decision date for this <br />application will be extended until March 2, 2007. <br />Rule 6.4.1 (11 Exhibit A -Legal Description <br />Rule 6.4.1 (1) requires identification of the location of the main entrance to the site. <br />Although we aze able to infer from other permit documentation the location of the main <br />entrance, please provide the location of the main entrance as specified in Rule 6.4.1 (1), <br />i.e., by latitude and longitude, or by the Universal Transverse Mercator (UTM) Grid. <br />Rule 6.4.3 (el Exhibit C -Pre-minine and Mining Plan Map(sl of Affected Lands <br />Exhibit C-1: Pre-Mine Plan Map, shows the locations of present vegetation covering the <br />affected lands, but no description of the type(s) of vegetation on the property.
