Outfall OOSA discharged each week of <br />the period. No permit exceedances were noted. <br />In order to reduce the amount of paper work, the following certification will be substituted for the actual DMR forms to cover <br />the ouffalls, which did not discharge. These ouffalls include ponds T (Ouffall 002A), B (Ouffall 003A), C (Ouffall 004A), E <br />(Outfall 006A), F (Outfali 007A), and G (Outfall 008A). Note that ponds A (Ouffall 001A), and H (Outfall 009A) have been <br />deactivated following bond release, and have been removed from the newly amended permit, effective January 1, 2003, Also <br />note that Ouffall 020A has not been constructed yet. <br />1 certify under penalty of law that 1 have personally examined and am familiar with the information submitted herein; and <br />based on my inquiry o/ those individuals responsible for obtaining the in/ormation, I believe the submitted information is <br />true, accurate, and complete. 1 am aware that there are significant penalties for submitting false information, including the <br />possibility of line and imprisonment. See 18 U.S.C. § 1001 and 33 U.S.C. § 1319.
