<br />2.03 PERMIT APPLICATION REQUIREMENTS <br />Rule <br />A completed Application Form for a Revision to a Coal Mining and Reclamation Permit is included as the <br />following page of this document. <br />2.03.1 Objective <br />This revision application addresses those items oz areas which have either substantially changed as a result <br />of permit revisions or the completion of mining and reclamation, may be relevant to future bond release <br />applications, or are no longer applicable (e.g. railroad loadout area). Only those sections of the permit <br />appbcation which have changed from the original application are included in this document. Any mle <br />requirement (of the Rules and Regulations of the DMG Pursuant to the Colorado Surface Coal Mining <br />Reclamation Act) not included or addressed in this document can be assumed to be unchanged from the <br />original application and is incorporated herein by reference as appropriate. Throughout this document, <br />all references to the original Hayden Gulch Mine pemut application (August 1980) will be in italics. <br />2.03.3 Fortnat and Supplemental Information <br />f <br />~ J <br />(1) Information in this permit application is current and supported by appropriate references to <br />technical and other written material available to the Division. It also includes revised permit <br />requirements as a result of the paztial Phase II/Phase III bond release. <br />1(a) A condensed version of selected information from this seven-volume permit application, as well <br />as post-mining topography and redamaton information can be found in Technical Revision No. <br />G3, dated August 1996. <br />1(b)Technical Revision No. 68, dated November 1999, reduced the pemtitted aaeage to 8.9 acres. <br />It also revised pemilt requirements as a result of the partial Phase II/Phase III bond release. TR- <br />68 is located at the front of this volume. <br />2. All tests, analyses and surveys carved out pursuant to these rules have been performed and/or <br />certified by a qualified person. <br />3, All chemical and physical laboratory analyses have been conducted by laboratories using approved <br />and standardized procedures. <br />.+} \C.0.
