The following items must be addressed by the applicant, in order to satisfy applicable requirements of C.R.S.
34-32.5 lOl et seq. and the Construction Materials Rules and Regulations of the Colorado Mined Land
Reclamation Board.

~~~ I. Rule 6.2.1 (2)(b) (Exhibit C D & F maps) requires that the preparer sign the maps. Please provide the
appropriate signature.
OCt 2. Rule 6.2.1(2)(e) requires that the exhibit C, D, & F maps have a legend. Provide such a legend.
ohs 3. On page 2 of the application state the present and future land use/s for the quarry portion of the
operations.
Exhibit A-Legal Description
Olti 4. The Iega1 description must also include that portion of the existing permit area (M-1981-079) that lies
west of the area described if it is to be included in the proposed permit area. The permit area boundary is
confusing in that area of the maps for Exhibits C,D, & F
Exhibit C-Pre-Mining and Mining Plan Map{s) of affected Lands
Ol^' S. During the pre-operation inspection Charles Murphy stated that Trout Creek will flow in a pipe across
the proposed permit area. If plans for the location of the pipeline are known show the location in this
exhibit and on the reclamation plan map, exhibit "E". This is not a requirement of your application;
however, if you can provide the information at this time, it will save you from having to revise the
permit, at a later date.
Ok 6. Rule 6.4.3(d) requires that you portrny the total area to be involved in the operation. This should be
done with an affected lands, as well as permit boundary. If these two boundaries are the same in certain
areas of the operation show them as such with an appropriate legend symbol or label on the maps. See
Rule 1:1(3) for the definition of affected land.
~1 7. The proposed permit and affected land boundaries are unclear in the area of existing Permit No.
M-81-079. Please clarify these boundaries.
