Neither <br />map contains a clearly labeled permit boundary. [f the heavy-lined polygon surrounding the Van 4 <br />Shaft area is the boundary, it should be labeled as such. The same symbol should also be used <br />around the half-acre parcel at the vent shaft to the east (and any access roads if needed). <br />Please clarify that the two areas at the east end of the claim block (on Curley Claim Nos. 4-6), <br />labeled "new mining areas" are underground only, and do not represent additional areas of surface <br />disturbance. <br />Please indicate on the map which surface structures are owned by others. This should at least <br />include all buildings, etc. owned by EFN, the specific electric utility structures owned by San <br />• Miguel Power, the county road, all fences, gates, cattleguards, etc. Please verify that all <br />applicable structures are depicted on the map too. (The oblique aerial photo you submitted shows <br />an extra powerpole which is not shown on the map.) <br />Some of these concerns may necessitate that a revision of these maps is submitted. <br />Exhibit F -Other Permits and Licenses <br />As mentioned above, a Department of Health Stormwater Permit may exist for this site. Will the <br />new operation of the site require a new replacement SWMP? Does the County require that a <br />permit for the new operation be obtained? <br />Exhibit l - Proof of Filing with the County Clerk and Recorder <br />Your recent submittal included a receipt dated 8/6/98 for the additional materials to be filed with <br />the county clerk. What is not clear is whether the other materials, originally filed fifteen months <br />prior, are still on file there. Please ensure that the complete and current application is on file, and <br />provide such evidence such evidence to this office. <br />Other Items <br />Please ensure that the newspaper publication has occurred. You must provide this office with an <br />affidavit of the publication and a copy of the notice before the decision date. Please provide a <br />copy of the notice soon though, so that this office may determine when the end of the public <br />comment is.
