• (Page 4) • <br />MINE ID # OR PP.OSPECTING ID ~' M-97-032 <br />• INSPECTION DATE 1/13/99 INSPECTOR'S INITIALS,~~~ <br />Scattered on the northern acre of the permit are about 250 cubic yards of waste rock, dumped <br />by loader or truck in small piles. This was probably done on top of the otherwise <br />undisturbed ground. Reclamation of this part of the permit will be by removal of the piles, <br />and placement on the contoured waste dump or in the shaft. It is assumed that level topsoil <br />is present underneath, which will require only revegetation. Until this is performed this <br />task will be included in the bond. <br />The vent shaft area is north of the county road. Its total disturbance easily fits into a <br />one-half acre block. At this point, the exact delineation of this block is not yet defined. <br />It should be described as a polygon of specific dimensions, and its boundaries marked, as the <br />main shaft area is. Surface features presently consist of an overhead powerline entering <br />from the west (via two IUC-owned poles from the utility company-owned line), a portable <br />electric control panel about 6 x 6 feet, a 15-foot diameter concrete shaft collar, a five- <br />foot vent fan placed over the shaft, and a few small debris items. No waste rock is <br />presently here or will be placed here until backfilling during reclamation occurs. Closure <br />may consist of complete backfilling, or of a bulkhead placed 50 feet underground with <br />backfill over it. The operator may wish to provide further details of the proposed method <br />of closure of this and other openings, in order to estimate a more accurate bond amount. <br />The second, or future, vent shaft is not yet constructed. It will be located east of the <br />existing one at an exact location to be determined later. It should be noted again that if <br />a road must be constructed or improved to allow access to this site, it must be included <br />within the permit area.
