Cattany <br />date: April 10, 2003 Division Director <br />The subject report was submitted on Apri17, 2003, and received at the Division on Apri19, 2003. <br />This review is conducted to determine compliance with Rule 4.05.9(17). <br />Rule 4.05.9(17) All impoundments shall be examined at least quarterly, by a qualifzed person <br />designated by the operators, for appearance of structural weakness2 and other hazardous <br />conditions'. The reports shall be retained at or near the mine site, and a copy of the report shall be <br />submitted to the Division on a quarterly basis", or other approved submittal frequency. <br />A qualified person, Jim Stover, a registered engineer, accomplished the inspection. <br />z The pond was inspected for structural weakness. There were none. <br />3 The pond inspected for other hazardous conditions. There were none. <br />The inspection was submitted to the Division on a quarterly basis.
