New forms are being sent to us, which we will fill out and send, until then we have made our own forms to <br />meet the deadline. <br />Please find enclosed the above referenced discharge monitoring report for the period October 1, 2000 through October 31, <br />2000. Ponds A (Outfall 001), B (Outfall 003), D (Outfall 005), and E (Outfall 006) discharged during this period. No permit <br />limitation exceedences occurred during this period. <br />Quarterly WET tests were conducted this month resulting in extra visits to Outfall 001 and Outtall 005, producing additional <br />field parameter values which were calculated into the statistics reported on the DMRS. <br />Outfalls 001, 005 and 007 are being tested for Hardness in addition to the required parameters, however, as noted below, 007 <br />did not discharge and was omitted for this period. A copy of those results is included in this submittal. <br />Outfalls 002, 004, 007, 006 and 009 did not discharge; and therefore have no chemical or toxicological tests during this period. <br />Outfall 020 has not been constructed yet; therefore there is no discharge to enable chemical or toxicological testing to date. <br />Outfalls 010, 016 and 017 have been eliminated from this permit. In order to reduce the amount of paper work, the following <br />certification will be substituted for the actual DMR forms fo cover the outfalls which did not discharge. <br />! certify under penalty of law that 1 have personally examined and am familiar with the inlormation submitted herein; anc <br />based on my inquiry of those individuals responsible for obtaining the inlormation, !believe the submitted information is <br />true, accurate, and complete. 1 am aware that there are significant penalties for submitting false information, including the <br />possibility of/ine and imprisonment, See 18 U.S.C. § 1001 and 33 U.S.C. § 1319.
