On February 19, 2003 we submitted a request to hold the application until modifications to the ~vlining and Reclamation Plans could be incorporated. In an effort to differentiate the updated application sections from the previous submittal we have enclosed one original whole anti complete unbound MLRB 112 permit application and 1 unbound copy. The application includes: mining approximately 89 acres of the 134.21 acre parcel, operation of batch plants, operation of a concrete/asphalt recycling facility and other mining associated functions at this site. A check in the amount of $2,344.00 for the applicatibn fee was initially submitted on December 31, 2003. Due to the changed in the plan, we have re-notified the local soil conservation distlrict and the Wetd County Board of County Commissioners; evidence of this is included in the application. As also required, this Application was submitted to the Weld County Clerk to the Board of County Commissioners. On the same date the DMG receives this Application a copy of the "Receipt of Application" signed by the Clerk will be forwarded via faz.
