Impact Operation (110) Reclamation Permit Application Division D~rermr
Dear Mr. Devereaux:
On June 21, 1995, the Division of Minerals and Geology received your 110(2)
application package for Costilla County's Sierra Grande Site, File No. M-94-103.
Preliminary review determined that the following items must be submitted before
the application can be considered submitted and technical review can begin:
EXHIBIT G - Source of Legal Rioht-to-Enter (Rule 6.3.7): The statement
submitted stating that Costilla County has the right-to-enter the property
and mine must be signed and notarized.
An affidavit or receipt indicating the date on which the
ion was placed with the local County Clerk and Recorder.
Conservation District (Rule 6.3.10): Proof of actual de2'ivery~or proof of
mailing by Certified Mail, return receipt requested; of Notice of the
Application to the local Board of County Commissioners and, if the mining
operation is within the boundaries of a Soil Co>servation District, to the
Hoard of Supervisors of the Soil Conservation District. Forms are enclosed
in the permit application.
Rule 1.6.2(1)(b) (Effective May 1, 1995): You must post notices at the
location of the proposed mine site, of sufficient size and number to
clearly identify the site as the location of a proposed mining operation
giving your name, address, and phone number, and stating that you have
applied for a mining permit with the Colorado Mined Land Reclamation Board.
Your notice must also state that anyone wishing to comment on the
application may view the application at the Fil.amoea County Clerk and
Recorder's Office and should send comments prior to the end of the public
comment period to the Colorado Mined Land Reclamation Division, Room 215,
1313 Sherman St, Denver, Colorado 80203.
After posting this notice you must provide a signed affidavit that such
notice has been posted according to the provisions of this Rule and submit
the off idavit to the Division.
You may amend the application by submitting all of the requested items. In
accordance with Rule 1.8, a new decision date will be established 30 days from
the date of receipt of all requested items. If you have already initiated
publication, you will need to republish notice which specifies the final date for
receiving comments is ten (10) days after the date of publication or the next
regular business day.
