<br /> <br />~ 1. Well Licence Application <br />~ Applications for well licences shall be made in duplicate on Form 1. Exact reproductions of Form <br />are acceptable. <br />S <br />I• 1.1. Application Form 1 <br />Applicants shall complete Form 1 in the following manner: <br />~ (a) all sections of the form shall be completed including the name of the drilling contractor (if <br />known at the time of application) except for sections labelled for Ministry use; and <br />~ (b) all numerical data shall be submitted in the International System of Units (SI). <br />1.2. Incomplete Applications <br />Incomplete applications will not be processed until missing information is provided. Missing <br />information may be submitted: <br />(a) by fax to (Fax 519-661-2809); or <br />(b) by editing electronically filed applications. <br />1.3. Application Exhibits <br />All applications must be accompanied by: <br />(a) for wells located: <br />(i) on land, a well location plan that complies with Part 2, <br />(ii) in water covered areas, a well location plan that complies with Part 2 that is submitted <br />with Form 7; <br />.~ (b) a drilling program that provides further details on the following: <br />~ (i) a geological prognosis of formation tops and expected oil, gas, water and loss of <br /> circulation zones and pressures; <br />~ (ii) drilling rig type(s); <br /> (iii) the hole size, casing size, grade, weight for the entire length of the well; <br />+~ (iv} a well bore schematic diagram showing expected casing setting points, hole and casinc <br /> size, how casing is set, water and hydrocarbon zones; <br />~ (v) contact personnel and phone numbers; <br /> (vi) drilling procedures described in sequence; <br /> (vii) casing and cementing procedures including cement type, additives, volumes and tops; <br />~ (viii) sample, coring, logging, testing and surveying programs; <br /> (ix) casing and formation integrity testing procedures; <br />','~ (x) drilling fluids; <br /> (xi) proposed alternatives or contingencies to accomplish isolation of the required formatior <br />= should the general program fail where specific ground or drilling conditions create <br />f problems e.g. lost circulation zones, aquifers, or other unusual conditions; and <br /> (c) the well licence application fee. <br />1.4. Application Fee <br />The application fee shall be in the form of a money order, cash, cheque orpre-authorized debit <br />from an account or credit card, or credit or debit card payable to the Minister of Finance, includinc <br />GST (unless exempted). <br />4 <br />
