Reoortine Requirements Reporting -Active Mining Operations: The permittee will be required to submit an Annual Report, covering January I through December 31 of each year, on their compliance with the SWMP. The Annual Report will contain, at a minimum: a) A repon on the facility's overall compliance with the SWMP. b) A summary of all comprehensive stormwater inspections made. c) Results and interpretation of any stormwater monitoring performed. The Annual Report will be due to the Division on or before February IS of each year (see Part /.E.1 of the permit). 2. Reporting -Inactive Mining Operations: The permittee will be required to submit a Compliance Report. This report will contain, at a minimum: a) A report on the facility's overall compliance with the SWMP. b) A summary or copies of the annual inspection reports, or the triennia! certification by a Professionaf Engineer that the facility is in compliance with the permit. The report will be due to the Division on or before February I5, 1999, and will cover the period of time from permit certification to December 31, 1998 (see Part 1.E.1 of the permit). Reporting - Remediation Projects: The permittee will be required to submit an Annual Report, covering January 1 through December 31 of each year, on the compliance with the SWMP. The Annual Report will contain, at a minimum: a) A report on the facility's overall compliance with the SWMP, including a progress repon on remediation activities. 6) A summary of al! comprehensive stormwater inspections made. c) Results and interpretation of any stormwater monitoring performed. The Arsnual Report will be due to the Division on or before February IS of each year (see Part I.E.1 of the permit). All reports required for submittal shall be signed and certified for accuracy by the permittee. (See Part I.E.2 of the permit.) E. Monitorine Sampling and testing of stormwater for specific parameters is not required on a routine basis under this permit. However, the Division reserves the right to require sampling and testing, on a case-by-case basis, in the event that there is reason to suspect that compliance with the SWMP is a problem, or ro measure the effectiveness of the BMPs in removing pollutants in the discharge. F. Retention of Records The permittee for active mining operations shall retain a copy of the SWMP required by this permit at the mining site for the duration of this permit. The permittee for inactive mining operations or mine-waste remediation sues shall retain a copy of the SWMP required by this permit, for the duration of this permit. For all types of operations, the permittee shall retain copies of all other reports required by this permit and records of all data used to complete the application to be covered by this permit, for a period of at least three years.
