Freauencv and Samole Tvpe (continued) <br />(d) Oil and Grease Monitorin¢ <br />For every outfall with oil and grease monitoring, in the event an oil sham or floating oil is observed, a grab sample <br />shall be collated, amlyred, and reported on the appropriate DMR. In addition, corrective action shall be taken <br />immediately to mitigate the discharge of oil and grease. A description of the corrective action taken should ba <br />included with the DMR. <br />2. Reoortin¢ of Data <br />Reporting of the data gathered in wmpliance with Part I.B.I shall be on a ttuatYerly basis. Monioring results shall be <br />summarised for each calendar quarter and reported on Division approved discharge monitoring report (DMR) forms <br />(EPA form 3320-1). The fortes shall be mailed to the agencies listed below so they are received no later than the 28th <br />day of the month following the end of the quarter (for example, the DMR for the first calendar quarter must be received <br />by the Division by April 28th). If no discharge occurs during the reporting period, 'No Discharge' shall be reported. <br />The DMR forms consist of four pages -the top 'original' copy, and three attached no~arbon-required copies. After dte <br />DMR form has been filled out end signed, the four copies must be separated and distributed as follows: <br />The first original signed copy of each discharge monitoring report (DMR) shall be submitted m the Division at the <br />• following address: <br />Colorado Department of Public Health and Environment <br />WQCD-PE-B2 <br />4300 Cherry Creek Drive South <br />Drover, Colorado 80222-1530 <br />The first duolirate signed copy of each discharge monitoring report (DMR) shall be submitted to the following agency: <br />U.S. Environmental Proration Agency <br />Water Management Division <br />NPDES Branch 8WM-C <br />999 18th Street, Suite 500 <br />Denver, CO 80202-2466 <br />The third end fourth copies are for the pecmittce records. The Discharge Monitoring Report forms shall be filled out <br />accurately and completely in accordance with requirements of this permit and the inswctions on the forms.
