Item 37 - Unless abeady covered by a stormwater permit, all applicants must complete the stonnwater application form included as Appendix C-I. No <br />other form is necessary. Questions relating to Appendices C-I and C-2 should be dvected to the stormwater group at (303) 692-3500. <br />PERMITTEES WHO PRESENTLY HAVE STORMWATER COVERAGE IN THEIR EXISTING PERMIT SHOULD NOT <br />COMPLETE APPENDIX C-1. SIMPLY SO NIDICATE AND SIGN THE SWMP CERTIFICATION STATEMENT W ITEM 37. <br />Item 38 - It is not required Iha[ [he applicant submit the Stormwater Management Plan (SWMP), however general permit applicants and applicants for <br />renewal of their stormwater permit must ceriify that a S WMP has been prepared by reading this item and signing as indicated. The Divisioa may <br />request a copy at a later date. A guidance document for S WMP preparation is included as Appendix C-2. Contact the stormwater group at (303) <br />692-3500 with any questions or assistance with the guidance. <br />Item 39,40 -The application shall be signed as follows: <br />a) in the case of a corporation, by a principal executive officer of at least the level ofvcecee-president or his or her duly authorized <br />representative, if such representative is responsible for We overall operation of the facr7hy from which the discharge described in the <br />application originates. <br />b) in the case of a partnership, by a general partner. <br />c) in the case of a sole proprietorship, by the proprietor. <br />d) In the case of a municipal, state, or other public facrb'ty, by either a principal executive officer, ranking elected official, or other duly <br />authorized employee. <br />Although ao advance copy of the application may be fazed, applications must be submitted with original sigoatu res to be processed. <br />Please coohct the Division for details. <br />
