30. Additional Monitoring: All applicants must review the paaameters listed in Appendix A and Appendix B to this application and indicate whether any of these pollutants aze known or believed to be present. For every pollutant expected to be discharged, the applicant must briefly describe the reasons the pollutant is expected m be discharged and report any quantimtive dam for the pollutant. CDMG Monitoring: Please submit a copy of summaries of all water quality monitoring dam of ou[falls or receiving streams for which dam has been gathered over the last five years for the mining site and which is requved by the Colorado Division of Minerals and Geology. If not aheady submitted m the Division, the plan which details the monitoring frequency, type, locations and method of analysis must also be submitted. !f these data are voluminous, please conmet the Division. Activity Durafion: When did the mining operation commence? 1982 Estimated life of the activity - minine has ceased years. 34. Pollution Prevention Plans: Please describe any pollution prevention or best management plans currently in place that could result m the improvement of water quality. This requvement may be waived by the Division if suimble dam on the dischazge's historic quality exists. 36. Additional Information: Please include any other information that you feel the Division should be aware of in drafting this permit. Include here any Bow or stream dam that you would Itke considered. Attach pages as gecessery. 37. Stormwater Discharges: All active and inactive coal mines must be covered by a stormwa[er permit. If your facility does not already have a Stormwater permit, complete Appendix C-1 and submit in addition to this application. ALSO, SEE ITEM 38. 38. Stormwater Management Plan: This item applies to all renewal perm8 applicants where the facility has Stormwater coverage through a combined process watedstormwater permit (individual or general) or through separate process water and Stormwater permits (and may apply to other facilities). In these cases, the permittee shall sign the fo0owing certification smtement. If a S WMP has not been prepared, see the S WMP guidance document in Appends C-2.
