Additional Notification Requirements: <br />Verbs[ norifica[ion shall be given to the Division within 24 hours ojrhe occurrence of any overJlou~ release. <br />A report mart be submitted within 5 days after the end of the release. The permittee must submit <br />documentatiors ojlhe storm event, including the precipitation gauge station (or the permittee's own gauge), <br />the estimated volume of nmoff produced, the status of infiltration or regeneration, and the steps taken to <br />minimize the amoum of overflow. <br />Regenerative releases, which are only authorized jot the lower pond at the Ironclad Overburden Storage <br />Area, must also be reported, but may be repored in a monthly summary, that will be submitted along with <br />the nest moNhly discharge monitoring report (DMR). The information submitted must include the same as <br />that listed above for overflow releases, as well as release rates and volumes, and the results of all analyses <br />performed in compliance with section V/.B. <br />3. hanger In Sources: <br />The permittee is required to visually inspect the base ojthe overburden storage area, the drainage area <br />downstream of the toe ojthe lower pond at point 003A, the base of the haul road, the toe ojthe pond <br />located below the haul road, and the area contributing Jlow to the pond at the crusher area. <br />Should any continuous seepage from these areas appear, or if there is visible evidence that uncontrolled <br />sediment discharge into waters ojthe Stare is occurring, the permit[ee is required to notify the Division in an <br />attachment that will be submitted along with the next monthly discharge monitoring report (DMR). <br />4. Soecial Reports -Special reports are required in rlre event of a spill, bypass, or other noncompliance. <br />Please refer to Par! /, Section D.4. of the permit jot reporting requirements. <br />G.
