NATURAL SODA, INC. <br />3200 County Rd. 31 Phone: (970) 878-3674 <br />Rifle, Colorado 81650 rax: (970) 878-5866 <br />March 28, 2005 <br />REC~t1~~i <br />Mr. Kent Walter, Area Manager <br />White River Resource Area MAR 2 9 2p05 <br />Bureau Of Land Management Division of Minerals and <br />73544 Highway 64 Geology <br />Meeker, CO 81641 <br />Subject: Notice of Intent to Abandon 91-1 V and 93-3 V <br />Dear Mr. Walter: <br />Enclosed please fmd Sundry Notices (two each) for production wells 91-1 V <br />(A.K.A. 26-91-1 V) and 93-3V (A.K.A. 26-93-3 V). A copy of `as-built" well <br />diagram and EPA form (Plugging and Abandonment} 7520-14 for each well are <br />also enclosed. The plugging and abandonment (P/A) plan meets or exceeds the <br />requirements in the Mine Plan and the UIC Permit. The proposed activity will <br />isolate aquifers and verify cement. Wells 91-1 V and 93-3 V were recovery wells <br />for cavities #2 and #3. P/A work was required by the UIC Permit. <br />Tn summary, a cast iron bridge plug (CIBP) will be set, a cement plug installed <br />from CIBP to top of annular cement. Cement will be tagged, casing perforated and <br />cement squeezed. Cement will be verified with cement bond log and procedure <br />repeated until aquifers are isolated. Finally, the annulus and well bore will be <br />cemented from surface and a surface hole location marker will be installed at <br />grade.
