Freauencv and Sample Tvoe (continued) <br />(n Oil and Grease ~lonitorin~ <br />For every outfall with oil and grease monitoring, in the event an oil sheen or floating oil is observed, a grab sample <br />shall be collected, analyzed, and reported on [he appropria[e DMR. In addition, correc[ive action shall be taken <br />immediately to mi[iga[e the discharge of oil and grease. A description of the correc[ive action taken should be <br />included with the DMR. <br />2. Reporting of Data <br />Reporting of the data gathered in compliance with Part I.B.1 shall be on a ua a basis. Monitoring results shall be <br />summarized for each calendar quaz[er and reported on Division approved discharge monitoring report (DMR) forms (EP.4 <br />form 3320-1). The forms shall be mailed to the agencies listed below so they are received no later than the 28th day of the <br />month following the end of the quarter (for example, the DMR for the firs[ calendar quarter mus[ be received by [he <br />Division by April 28th). If no discharge occurs during the reporting period, 'No Discharge' shall be reported. <br />The DIvIR forms consis[ of four pages - [he top 'original" copy, and [hree attached no-carbon-required copies. The Discharge Monitoring Report forms shall be filled out <br />accura[ely and completely in accordance wiilt requirements of [his permit and the instructions on the forms. Acute WET Testing - Outfalls 011. 0l3 and 017 <br />(a) Testing and Reporting Requiremen[s <br />Tests shall be done a[ the frequency listed in Part I.B.I.
