Erequ~y and Sample Type (continued) <br />(~ Oil and Grease Monitorine <br />For every outfall with oil and grease monitoring, in the event an oil sheen or floating oil is observed, a grab sample <br />shall be collected, analyzed, and reported on the appropriate DMR. In addition, corrective action shall be [aken <br />immediately to mi[iga[e the discharge of oil and grease. A description of the corrective action taken should be <br />included with the DMR. <br />2. Reuortinpyf Data <br />Reporting of the data gathered in compliance with Pan I.B.1 shall be on a quarterly basis. Monitoring results shall be <br />summarized for each calendar quaver and reported on Division approved discharge monitoring report (DMR) forms (EPA <br />form 3320-1). The forms shall be mailed to the agencies listed below so they are received no later than the 28th day of the <br />month following the end of [he quarter (for example, the DMR for the first calendar quaver must be received by the <br />Division by April 28th). If no discharge occurs during the repotting period, "No Discharge" shall be reported. <br />The DMR forms consist of four pages -the top "original" copy, and three attached no-carbon-required copies. After the <br />DMR form has been filled out and signed, the four copies must be separa[ed and dis[ributed as follows: <br />The first original signed copy of each discharge monitoring report (DMR) shall be submitted to the Division at the <br />following address: <br />• Colorado Department of Public Health and Environment <br />WQCD-PE-B2 <br />4300 Cherry Creek Drive South <br />Denver. CO 80222-1530 <br />The first duplicate signed copy of each discharge monitoring report (DMR) shall be submitted [o the following agency: <br />U.S. Environmental Protection Agency <br />Technical Enforcement Program SENF-T <br />Office of Enforcement, Compliance Assistance and Environtental Justice <br />999 l8th Street, Suite 500 <br />Denver, CO 80202-2466 <br />The third and fourth copies are for the permit[ee records. The Discharge Monitoring Report forms shall be filled out <br />accurately and completely in accordance with requirements of this penni[ and the ins[ructions on the forms. They shall be <br />signed by an authorized person as iden[ified in Pan 1.D. <br />3. Acute WET Testing - Outfalls 002. 004. Oll and Ol3 <br />(a) Testing and ReportinE Requirements <br />Tess shall be done at the frequency listed in Part I.B.I. Test results shall be reported along with the Discharge <br />Monitoring Report (DMR) submitted for the repotting period during which the sample was taken. (i.e., WET testing <br />results for the first calendar quaver ending Mazch 31 shall be repotted with the DMR due April 28.) The results shall <br />be submitted on the Acute Toziciry Test report form, available from the Division. Copies of these reports are to be <br />submitted to both the Division and EPA along with dte DMR. <br />See Part i.C. for Definitions.
