Erequ~y and Sample Type (continued) (~ Oil and Grease Monitorine For every outfall with oil and grease monitoring, in the event an oil sheen or floating oil is observed, a grab sample shall be collected, analyzed, and reported on the appropriate DMR. In addition, corrective action shall be [aken immediately to mi[iga[e the discharge of oil and grease. A description of the corrective action taken should be included with the DMR. 2. Reuortinpyf Data Reporting of the data gathered in compliance with Pan I.B.1 shall be on a quarterly basis. Monitoring results shall be summarized for each calendar quaver and reported on Division approved discharge monitoring report (DMR) forms (EPA form 3320-1). The forms shall be mailed to the agencies listed below so they are received no later than the 28th day of the month following the end of [he quarter (for example, the DMR for the first calendar quaver must be received by the Division by April 28th). If no discharge occurs during the repotting period, "No Discharge" shall be reported. The DMR forms consist of four pages -the top "original" copy, and three attached no-carbon-required copies. After the DMR form has been filled out and signed, the four copies must be separa[ed and dis[ributed as follows: The first original signed copy of each discharge monitoring report (DMR) shall be submitted to the Division at the following address: • Colorado Department of Public Health and Environment WQCD-PE-B2 4300 Cherry Creek Drive South Denver. CO 80222-1530 The first duplicate signed copy of each discharge monitoring report (DMR) shall be submitted [o the following agency: U.S. Environmental Protection Agency Technical Enforcement Program SENF-T Office of Enforcement, Compliance Assistance and Environtental Justice 999 l8th Street, Suite 500 Denver, CO 80202-2466 The third and fourth copies are for the permit[ee records. The Discharge Monitoring Report forms shall be filled out accurately and completely in accordance with requirements of this penni[ and the ins[ructions on the forms. They shall be signed by an authorized person as iden[ified in Pan 1.D. 3. Acute WET Testing - Outfalls 002. 004. Oll and Ol3 (a) Testing and ReportinE Requirements Tess shall be done at the frequency listed in Part I.B.I. Test results shall be reported along with the Discharge Monitoring Report (DMR) submitted for the repotting period during which the sample was taken. (i.e., WET testing results for the first calendar quaver ending Mazch 31 shall be repotted with the DMR due April 28.) The results shall be submitted on the Acute Toziciry Test report form, available from the Division. Copies of these reports are to be submitted to both the Division and EPA along with dte DMR.
