PARTI <br />Page No.:10 <br />Permit No. COG-850000 <br />D. GENERAL MONITORING, SAMPLING, AND REPORTING REQUIItEMENTS _" ".C i - F ~ 1 : ~?'' <br />1. Routine Reoortine of Data ~ ~ - <br />Reporting of the data gathered in compliance with Part I.B.3. shall be on a quarterly basis. Reporting of all data gathered shall coral <br />with the requirements of Part I.D. (General Requirements). Monitoring results shall be summarized for each calendaz quarter and <br />reported on Division approved dischazge monitoring report (DMR) forms (EPA form 3320-1). The forms shall be mailed to the agencies <br />listed below so they aze received no later than the 28th day of the month following the end of the quarter (for example, the DMR fot the <br />first calendaz quarter must be received by the Division by April 28th). If no dischazge occlas during the reporting period,'No Discharge" <br />shall be reported. <br />The DMR forms consist of four pages -the top "original" copy, and tbreeattached no-cazbon-required copies. After [he DMR form has <br />been filled out and signed, the four copies must be separated and distributed as follows: <br />The fast original signed copy of each dischazge titorritoring report (DMR) shall be submitted to the Division at the following address: <br />Colorado Department of Public Health and Environment <br />Water Quality Control Division <br />WQCD-P-B2 <br />4300 Cherry Creek Drive South <br />Denver, CO 80222-1530 <br />The US Environmental Protection Agency Region VIII no longer requires submittal of duplicate copies of the DMR to their office, <br />therefore, the second, third and fourth copies aze for the perttrittee records. The Dischazge Monitoring Report forms shall be filled out <br />accurately and completely in accordance with requirements of this permit and the instmctions on the fortes. An authorized person as <br />identified in Part I.D.7 shall sign them <br />Calculations for all limitations, which require the averaging of measurements, shall utilize an arithmetic mean unless otherwise specified <br />by the Division in the permit. <br />2. Reporting - Stormwater Annual Report <br />The permittee will be required to submit an Annual Report, covering January I through December 31 of each yeaz, on the ove: <br />compliance with dte SWMP. The Annual Report will contain, at a minimum: <br />a) Name of permittee, address, phone number, and permit certification number. <br />b) A report on the facility's overall corr~liance with the SWMP. <br />c) A summary of each comprehensive stormwater facility inspection made, including date, findings, and action taken. <br />d) Results and interpretation of any stormwater monitoring performed. <br />e) The report shall be signed and certified for acctuacy by the petmittee, including the certification language contained in Part I.D.7. of <br />the permit. <br />The Annual Report will be due to the Division on or before February I S of each yeaz, after the first full yeaz of coverage underthe permit. <br />The exact due date for the petmittee's fast Annual Report will be listed in their permit certification. Representative Sampline <br />Samples and measurements taken as required herein shall be representative of the volume and nature of the monitored discharge. All <br />samples shall be taken at the monitoring points specified in this permit and, unless otherwise specified, before the effluent joins or is <br />diluted by any other wastestteam, body of water, or substance. Monitoring points shall not be changed without notification to and <br />approval by the Division. <br />If the per~ttee monitors at the point of dischazge any pollutant limited by the permit more frequently than required by the permit, us' <br />approved test procedures or as specified in the permit, the result of this monitoring shall be included in the calculation and reporting <br />data to the Division. <br />
