TETRATEGH RMC <br />April 2, 2004 <br />Mr. Mike Refer <br />Aggregate Industries, Inc. <br />1707 Cole Boulevazd, Suite 100 <br />Golden, Colorado 80401 <br />Re: Well Inventory, Hazeltine Aggregate Site Near Henderson, Colorado; <br />Tetra Tech RMC Job No. 19-3919.018.00 <br />Dear Mike: <br />This letter report presents the results of our well inventory for the proposed Hazeltine Aggregate <br />Mine near Henderson, Colorado. Aggregate Industries (AI) is in the process of applying for a permit <br />to extract gravel at the Hazeltine site. Reconnaissance of the site and adjacent areas indicated the <br />presence of numerous, shallow, domestic wells. Review of well records on file with the Colorado <br />Department of Natural Resources, Office of the State Engineer (SEO) also confirmed many domestic <br />wells are present near the site. Experience has shown that the SEO domestic well files are not <br />complete. Consequently, AI conducted a well inventory to identify, to the extent practical, domestic <br />wells near the proposed site. <br />The search area of the well inventory was 1,500 feet from the property boundary. This distance was <br />chosen because impacts to groundwater levels are expected to be limited to within 1,000 to 1,200 feet <br />of dewater cells. The distance of 1,500 feet was chosen to be conservative. Wells identified during <br />the inventory are summarized in Table 1. Table 1 is subdivided into two sections (A and B), by <br />distance: wells within 600 feet of the property boundary are summarized in Table 1-A, and wells <br />between 600 and 1,500 feet of the properly boundary are summarized in Table 1-B. The <br />approximate locations of the wells are shown on Figure I. <br />WELL INVENTORY <br />The well inventory process consisted of several key elements which are described below. <br />A letter and questionnaire were prepared for delivery to all residences within the inventory area. <br />Copies of the letter and questionnaire are provided in Appendix A. The letter and questionnaire, <br />along with a self addressed postage paid envelope, were hand delivered to each residence in the <br />inventory area. The letters were delivered during the first week of November 2003. If a resident was <br />home when the letter was delivered, the Tetra Tech RMC representative interviewed the resident to <br />complete the information on the questionnaire. If [he resident was not home, the information was Left <br />on the door knob or other conspicuous place. <br />1900 S.
